COVID-19 LIABILITY WAIVER FOR MINOR

I am the parent or legal guardian of (hereinafter referred to as Minor).

I hereby release, waive, and forever discharge any and all liability, claims, and/or demands of whatever kind or nature in either law or equity against Best Martial Arts Institute and all of its instructors, volunteers, officers, directors, employees, partners, sponsors, and agents (the “Released Parties”) resulting from Minor’s exposure to and/or contraction of the COVID-19 virus while partaking of in-person group activities sponsored by the Released Parties to the fullest extent allowable by law. I hereby waive both my right and Minor’s right to recover any and all economic losses, damages, and/or out-of-pocket expenses resulting from Minor’s exposure to and/or contraction of the COVID-19 virus and release the Released Parties from any and all such liability. These provisions are binding on Minor’s heirs, legal representatives, successors, and assigns.

I hereby acknowledge the following:

1. Participation in in-person group activities, whether indoor or outdoor, includes possible exposure to, and illness from, COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and

2. On behalf of Minor, I knowingly and freely assume the risk of injury, harm, and/or loss associated with the Released Parties’ in-person activities, including any injury, harm, and/or loss caused by the negligence, fault, or conduct by the Released Parties.

STATEMENT OF COVID-19 VACCINATION

I attest that Minor has completed a COVID-19 vaccination course and I have submitted a true copy of their COVID-19 Vaccination Record Card with this signed liability waiver.
