Application for Martial Arts Demonstration

Demonstrations will run on a dedicated martial arts stage from 10:15 a.m. to 4:30.
The stage size is 20' x 40'. The stage will be raised about 2' off the ground.
Demonstrations will be a maximum of 25-minutes long, with a 5-minute set-up time between acts.

RELEASE AGREEMENT: In consideration of the granting of permission to participate in the Oregon Asian Celebration, I, the undersigned, intending to legally bind hereby myself, heirs, personal representatives and all volunteers and agents of the organization I represent, agree to waive, discharge and release any and all rights and claims for injuries, losses and damages against the Oregon Asian Celebration and/or the Asian Council, their officers, employees, agents and volunteers from any and all injuries, losses or damages to my personal property and the property of the organization I represent including theft thereof, or for any injury or pain to personnel, participants and/or customers.
